Outpatient Physical Therapy Services with Texas Physical Therapy Specialists Criteria:

· Current up to date MAP eligibility
· Back Pain
· Neck Pain
· Fall risk
· Deconditioning
· Post-cardiac patients who can be safely served in an outpatient setting
· Cancer treatment or survivorship with mobility challenges
· Mild lymphedema
· Any musculoskeletal complaint, including but not limited to knee, hip, elbow and shoulder pain
· Gait difficulties
